• Run a narrow test to reproduce (state R) and record non‑zero exit
• Apply patch and verify it passes (state V) with zero exit
• Record states in a ledger
• Add a verifier to CI that checks ledger states
• Optionally reverse patch and rerun R to confirm coverage
